Jotform Account Setup & Usage
Welcome to Superclean Maid Service! We are glad to have you on the team.
As part of our compliance with our partner aged care providers, all staff must complete the Superclean Client Visit Form when servicing aged care clients. This form is submitted through the Jotform app and serves as proof that the service was completed, allowing us to invoice our aged care providers correctly.
Please carefully follow the instructions below step by step:
Step 1: Please download Jotform app and create your own account using your preferred email address:
Step 2: Once you have created your account, please request access to the Superclean Client Visit form:
Step 3: After requesting access, you will receive an email invite that allows you to access the Superclean Client Visit form on your Jotform app. Please wait up to 1 business day to receive the email.
Once you receive it, please click “View Form” to accept the invite.
Step 4: Go back to your Jotform app. You should now have access to the Superclean Client Visit form. To check, click “More” at the bottom right, and then click “assigned to me“.
Step 5: Ensure the Superclean Client Visit form is available on offline mode. To do this, click the form and tick “offline mode“.
