LCHS QR Code System
As part of our requirements with Latrobe Community Health Service (LCHS), there is now a mandatory QR code check-in process that must be followed when servicing LCHS participants.
As part of your induction, please read and understand the process below before your first LCHS shift. This is a mandatory requirement.
What You Need to Do at Each LCHS Job
When you arrive at a participant’s home:
Scan the QR code at the participant’s home using your mobile phone.
This will open a webpage where you log in to start the service.
When you finish the job, return to the same webpage and stop the service.
This system records the time and duration of the service you provided.
End-of-Shift Questions
When you stop the service, you will be asked three questions about the participant’s health.
Please answer honestly and add any relevant comments.
If you did not see the participant (for example, you were doing gardening and they weren’t home), write:
“Participant Not Home.”
Where to Find the QR Code
Indoor workers: The QR code sticker should be on the fridge.
Outdoor workers: The QR code sticker should be on a window.

Please make sure you follow this process exactly, as it is required for compliance and payment processing.
